The new client paperwork is for everyone. Please print in color. If you can't print in color, we'll have you sign one page in color again at the office. 

If you'd like to use insurance, please fill out the Insurance Verification Guide. We require all clients to call their insurance company and get the most accurate information possible. For example, Medicare covers acupuncture, but does not cover if it's done by a private practice acupuncturist. We want to be sure all parties are on the same page about the coverage.
